$200 for each additional child, up to 3 children 

No additional charge after 4th child 

No surcharge for Point of Care Testing (i.e. Rapid Strep, Rapid Covid Antigen, Urinalysis)

No surcharge for medication administered during visit (i.e. nebulized albuterol, steroids)
